CONSENT FORM

All players NEW to NYSA are required to provide a copy of their birth certificate upon registration.
Registrations will be accepted on a first-come, first-serve basis during open registration dates.
Teams are typically limited to 18 players; teams may be over-rostered at a maximum of 22 players. Over-rostering will be done at the discretion of the Registrar.
Team placement will be made according to birthdate guidelines. Players may be asked to “play-up” ONLY if they are needed to fill a team.
Teams will be assigned to play under either the Arrowhead Youth Soccer Association (AYSA) or the Minnesota Youth Soccer Association (MYSA). This assignment will be made at the discretion of the Northwoods Association Board and/or team coaches.
There is potential for any team to be declared a “tournament-only” team.
All teams playing under AYSA will be required to travel to Duluth and the surrounding area.
All teams playing under MYSA will be required to travel to the Twin Cities Metro Area.
It may be required of any team moving through to their District tournament or beyond to extend their season to accommodate such obligations. Such a decision will be made among the coaches, players, parents, and NYSA board.
A high level of commitment is required by each player to complete the teams’ standards of play; accountability is a must.
Each player must be represented by a parent/guardian at a pre-season parent meeting as arranged per team.
Players will be required to abide by the NYSA Substance Abuse Policy.
In accordance with AYSA policies, tobacco use is not allowed at games or practices; the no-smoking areas include field parking lots.
Parents are strongly encouraged to transport players to and from games and practices in smoke-free vehicles.
Families will be encouraged to provide a monetary contribution to assist in covering any lodging expenses the team’s coach may incur while the team is playing outside of the Grand Rapids area.
Partial refunds will be given for medical reasons only before start of the season; a doctor’s note is required.
Players needing financial assistance with participation fees are encouraged to approach the board with a letter of financial need.
I, the undersigned, grant permission and consent to NYSA the right to record, use, reproduce, and distribute my voice, image, and name in audio, video, photography and other multimedia formats for the purpose of promoting youth soccer.
I understand that NYSA will control the copyright of this material (photographs, video, audio recording, and other multimedia formats).
I understand that this material is for use by NYSA so I waive any right to inspect or approve any proposed use of the multimedia material in any medium such as printed brochures, television advertisements, internet websites, newspaper articles, radio programs, etc.
I agree that my participation in the multimedia promotion of youth soccer by NYSA is on a volunteer basis without any compensation and hereby waive any payment or rights to royalties.
I hereby release NYSA from any liability in connection with the multimedia promotion of youth soccer.
I state that I am at least 18 years of age and am competent to contract in my name. A parent or guardian must sign for all participants under the age of 18.
I have read and understand this CONSENT FORM and acknowledge that it is firm and final and that NYSA may proceed in reliance thereon.
